LSR Mission Statement:
It is our mission to provide training and support to individuals with disabilities so they may achieve maximum potential for self-reliance and active participation in the community.
Lincoln Self Reliance, Inc. has been in operation as a Section 501(c)(3) corporation since July 2000. LSR, Inc. maintains physical facilities in Diamondville, Kemmerer, Afton, Thayne, and Evanston Wyoming. The Administration Office is located in Afton Wyoming.
Over the past fiscal year, the Evanston programs have continued to receive a lot of physical facilities updates. A new kitchen (cabinets, countertop, sink, dishwasher, wiring and plumbing) was installed at DayHab-A located at 100 Bear River Drive. Prior to this addition the two DayHab programs were sharing one kitchen. Support staff and persons served were thrilled with the new kitchen. To better support individuals with mobility issues LSR had a tub/shower located in a changing room at 100 Bear River Drive, replaced with a wheelchair accessibility shower. LSR also replaced a wooden ramp at the Champs residential home with a concrete ramp with steel railings.
The Kemmerer/Diamondville programs experienced a slight reduction in persons served. Two long term individuals served moved away from the Kemmerer area. Staff and persons served were sad to see them leave and hope the best for their future endeavors. As a result of one of the individuals moving away, the Kemmerer/Diamondville location is no longer running a document destruction program.
LSR’s Thrift Store located at the 1038 South Washington had another successful year. This past fiscal year LSR was able to increase the number of hours that persons served in the Afton area are able to receive paid work at the Thrift Store. The community continues to be a huge supporter of the store and as a result a tremendous amount of clothing and furniture donations have been received. Revenue from the Thrift Store for the fiscal year was $76,150.
LSR’s storage building located at 720 Adams St., Afton, received some much needed attention. The building is used to store seasonal Thrift Store donations and equipment used at LSR. Thomas Skinner took on the task as an Eagle Scout Project. With assistance from his troop and family the storage building received a much needed new coat of paint.

LSR, Inc. maintains a Governance Board of five to ten members from various backgrounds and communities in Lincoln County. LSR, Inc. employs an Executive Director, Darrel Skinner, MSW, to manage the day-to-day operations of the organization. Board members serving at the close of the 2016-2017 fiscal year were:

LSR, Inc. maintains CARF accreditation for Community Integration; Community Housing; Supported Living; and, Community Employment Services: Employment Supports. LSR received another Three Year Accreditation at the conclusion of our accreditation survey conducted in November 2016. “CARF accreditation is a process that demonstrates a provider has met standards for the quality of its services. CARF established these standards to guide providers in offering their services. CARF also uses the standards to evaluate how well a provider is serving people and how it can improve.”
Programs and Services
During the fiscal year ending June 30, 2017, LSR provided services to 71 individuals.
• Thirty individuals received Residential Habilitation services through LSR.
• Fifty-seven individuals received Day Programming/Community Integration Services through LSR.
• LSR also provided supported living, personal care, supported employment, respite, companion services, adaptive equipment, support brokerage and intervention services during the fiscal year.
• In a Satisfaction Survey conducted in December 2016, stakeholders indicated 100% overall satisfaction with DayHab services; 100% satisfaction with Residential Services.
• LSR did not have any formal grievances filed by clients or stakeholders during the fiscal year.
• Of all individuals served in adult services 63% participated in paid or volunteer work.
• In an employee Satisfaction Survey conducted in December 2016, respondents indicated 84% employment satisfaction working for LSR.

Celebration of Achievements:
An exciting evening was enjoyed by all who attended the annual awards banquet held at the Afton Civic Center, Kemmerer Events Center, and Evanston Round House. All persons receiving services with LSR received recognition for accomplishments throughout the past year. Community Service Awards were presented to:

• Ridley’s Family Market – Diamondville/Kemmerer Community Service Award
• Best Home Health – Evanston Community Service Award
On June 30, 2016, LSR employed one hundred eleven (111) staff. LSR also employed ten (10) persons served on-site.

A financial audit of LSR records for the period July 1, 2016 to June 30, 2017 was completed by DeCoria & Company, Certified Public Accounts. The audit found no significant financial irregularities for the fiscal year.
